Only the way to stay and work in Japan for a long term is you must get the intentional license during having special working permit for 4 or 5 years.
In the case of a caregiver, you must get a Japan caregiver license within 4 or 5 years.

You have an opportunity to apply to Immigration to switch working permit (Caregiver VISA).

During work for the caregiver, you can have it.
